Home and Community Care Program

The Home and Community Care Program assists clients to live in the community as independently as possible. Clients who live on reserve are assessed for services by the Home and Community Care Nurse. Once the initial assessment is completed a client care plan is developed for the client to receive essential services based on their specific needs. Home and Community Care is not intended to replace care provided by family rather to assist in providing for client care needs.

Services provided by the Home and Community Care Program consist of the following;

1. Nursing: includes in home nursing care and coordination of care services.
2. Home Management: includes a general cleaning of client’s home, sweep/vacuum, wash floors, washing dishes, wash bathroom, clean stove, fridge and bedrooms.
3. Personal Care: includes personal hygiene assistance, bathing, grooming, assistance with transfers/mobility, assistance with toileting, dressing
4. Reassurance Visits: includes systematic checks with individuals, usually through daily visits or telephone calls to ensure that clients are managing satisfactorily.
5. Respite Care: supervision of and assistance to clients who cannot be left alone.
6. Meal Service: includes meal preparation in clients’ home.
7. Seasonal Cleaning: thorough cleaning of clients’ home

All staff are required to maintain accurate records, ensuring proper reporting on the provision of client care. Monthly statistical reporting is done, to date we have a total of 23 clients who receive care.

Home and Community Care staffs 5 employees, Home and Community Care Nurse and Supervisor, Home and Community Care Coordinator, 2 Certified Home Health Aides and a Home and Community Care Homemaker.
