Community Health Representative (CHR): Linda Head

The CHR works with the Community Health Nurse in providing services to/for:

  • Elderly
  • Chronic care clients & diabetics
  • Environmental Health – informing community of boiled water advisories
  • Programming in monthly health themes
  • Well baby clinics
  • CPNP
  • School age immunizations (obtaining consents, etc)
  • Preschool screening
  • TB screening
  • Community Kitchen programs
  • Pre and post natal follow-ups
  • Assisting with Emergency Preparedness/Pandemic Planning
  • Assisting Dietitian with workshops and scheduling counseling sessions
  • Home visits

 

2. Maternal Child Health Program

Promotes healthy outcomes in pregnancy, pre and postnatally.

The Home Care program assists community members to live as independently as possible.

6. Medical Transportation/Reception
