1.0 Introduction:

Education for Mistawasis First Nation #103 peoples fosters a continuous holistic journey along the circle of life. It is rooted in the uniqueness associated with that of being First Nations people and is based on the premise of respect for all life and all things.

VISION:

Mistawasis First Nation #103 Chief and Council will support its members, who sincerely demonstrate that they will benefit from education that promotes personal growth and development resulting in a successful, independent lifestyle and success in their chosen field of endeavor.

Education for Mistawasis First Nation #103 is guided by the following principles:

Consistency Equity Openness
Clarity Respect Visibility

Each and every member of the Mistawasis First Nation #103 is guaranteed the right to access education “for as long as the grass grows, the rivers flow, and the sun shines” as specified and agreed upon by the signing of Treaty 6 by the representatives of the Plains Cree First Nations and the Crown.

The Mistawasis First Nation #103 is diligent, and will remain so, in advancing the recognition of these rights. It is the unfailing belief of the Mistawasis First Nation #103 that this right to access education encompasses all levels of educational pursuit and is the Chattel of Treaty Indians regardless of residency.

Mistawsis First Nation asserts that Post Secondary Education is a Treaty Right and nothing in its acceptance of administrative responsibility for the Post Secondary Student Support Program (PSSSP) abridges that Treaty Right nor the trust responsibility of the Government of Canada towards Treaty First Nations.

The Post Secondary Student Support Program supports Treaty/Status Indian Students pursuing post secondary studies in recognized and authorized post secondary institutions. The objective of the Post Secondary Student Support Program are to encourage and support pre-qualified, eligible students to acquire college, technical institute, or university to acquire professional qualifications so that they can become economically self sufficient and develop their own individual potential to further their own goals. This manuel provides policy direction for Mistawsis administration of the PSSSP and where appropriate, forms the basis for operating guidelines for the program student and staff.

2.0 Organizational Structure:

The organizational chart approved by the Mistawasis First Nation #103 Chief and Council shows the formal reporting and supervision responsibilities of all members. To follow protocol is the accepted and correct way of doing things and adhering to the system that has been put in place for all. In order for the system to work properly, all members shall follow protocol:

The primary obligation of Mistawasis First Nation #103 Chief and Council is toward all members under its jurisdiction. The most important activity of Chief and Council is to formulate policies that promote the delivery of efficient and quality services to its members.

The main purpose of this policy is to provide efficient and quality educational assistance for all Mistawasis First Nation #103 members. It is the intent of the Chief and Council to have policies, guidelines, and procedures that comply with the Department of Indian Affairs, Federal statutes and regulations.

3.0 Definitions:

3.1 “Treaty/Status Indian” means a person whose name has been entered in the Indian register in accordance with Indian Act and/or a person recognized as possessing Band membership status according to the Mistawasis First Nation #103.

3.2 “Post Secondary Education” means a program of studies offered by a recognized post-secondary institution, for which completion of secondary school (Grade 12) or its equivalent is normally a prerequisite.

3.3 “Program of Studies” includes all post-secondary programs leading to a certificate, diploma or degree.

3.4 “Post Secondary Institutions” are degree, diploma and certificate-granting institutions that are recognized by both provincial and federal authorities and include educational institutions affiliated with or delivering accredited programs by arrangement with a post-secondary institution. Mistawasis First Nation #103 will maintain a list of recognized Canadian post-secondary institutions.

3.5 “Public Institution” is a post-secondary institution that receives the majority of its funding from federal and provincial governments.

3.6 “Private Institution” is a post-secondary institution that receives the majority of its funding from sources other than governments.

3.7 “Full time Student” as defined and in accordance with the Mistawasis First Nation #103 Post-Secondary Policy.

3.8 “Academic year” as defined by the post-secondary institution.

3.9 “Semester” refers to a part of the academic year, as defined by the post-secondary institution. Semesters usually cover the periods: January - April, Intersession (May - August), and September - December.

3.10 “Dependent Spouse” refers to a person who is married to the student and is registered in Mistawasis First Nation #103 Indian Registry (refer to Sec. 3.1). This person is financially dependent upon the student and does not receive an income allowed for a dependant spouse by Revenue Canada. If a student claims a spouse and/or dependant, legal documentation must be provided through the administering organization before living allowance rates are increased. i.e. Commissioner of Oaths and health cards, etc.

3.11 “Dependent” means the child who is registered in Mistawasis First Nation #103 Indian Registry (refer to Sec. 3.1). The child is solely and financially dependent upon the student and resides with the family and attending school full-time.

3.12 “Immediate Family” includes spouse, mother, father, sister, brother, son/daughter, mother-in-law, father-in-law, step parents, maternal and paternal grandparents or any relative residing in the student’s household or with whom the student resides.

3.13 “Emergency/Contingency” refers to emergency assistance that must be valid and verifiable.

3.14 “Special Needs” refers to funded student and/or dependent with a disability, who requires special need costs.

3.15 “PSSSP” refers to the Mistawasis Post-Secondary Student Support Program.

4.0 Application Process:

The student must be a band member of Mistawasis First Nation for whom Mistawasis administers the PSSSP and this includes Bill C-31.

4.1 Applying to the Post-Secondary Institution:

a) Applying for enrollment to the post-secondary institution and program, in compliance to the rules set by the institute, is the student’s responsibility. Help is available by contacting the Post-Secondary Education Office from 8:30 am - 4:30 pm, Monday to Friday.

b) Applying for transcripts required by submission by the post-secondary institution and program is the student’s responsibility.

4.2 Applying for PSSSP (Post-Sec Student Support Program) Funding:

a) Deadline dates - fully completed application form and all related documentation must be received at the Mistawasis Post-Sec Office prior to the following deadline dates:

June 15 Fall semester (September - December)
October 15 Winter semester (January - April)
March 15 Intersession (May - August)

** Short courses - one month prior to start date.

b) PSSSP funding application forms are available at either the Mistawasis Post-Sec Office at: Mistawasis Post-Secondary Education Office

c) All related documentation:
All applicants must provide a letter of acceptance from the institution, copies of transcripts from secondary and post-secondary institutes last attended (refer to Sec. 4.2a).

d) It is considered “fraud” if a student knowingly misrepresents information in their post-secondary forms. Where information provided by the student is found to be false, the policy will be applied to the fullest extent. The student will then be asked to sign an affidavit.

e) All applications received by the Mistawasis Post-Secondary Education Office will be recorded and a letter of acknowledgement will be sent to the applicant.

5.0 Eligibility for Post-Secondary Student Support:

5.1 Fully completed application form and all documentation prior to deadline dates specified in Section 4.2a and c.

5.2 Applicants who are conditionally accepted with deficiencies by the post-secondary institute and/or program, must clear those deficiencies as per letter of conditional acceptance before application for funding can be considered.

5.3 Support will be provided within the limits of funds available in accordance with Mistawasis funding arrangement. If demand for funding exceeds availability, applications will be deferred according to priorities set out in Section 6.

5.4 If demand for funding exceeds availability after October 15, all deferred applicants must re-apply for sponsorship annually. It is the applicant’s responsibility to keep informed of application deadline dates and eligibility criteria requirements for funding.

5.5 Applicants taking short courses: funding will be determined on the availability of financial resources at the time of the request. Applicants must comply with the rules outlined in Section 4 and submit request for funding one month prior to start date of program.

5.6 Mistawasis Post-Secondary Education Office will advise all applicants regarding their status of sponsorship, at the earliest possible time.

6.0 Priorities for Approval of Applications:

The following categories may be modified based on the types of applications received for any specific intake. Priorities for approval for student applications shall be based on the following:
Group 1 is the first set of applications to be approved.
Group 2 next in priority
Group 3

GROUP 1 - These categories will be considered equal:

1. Continuing students who maintain grade average requirements of the program/institute.
2. Regular academic Grade 12, including ABE 12, entering directly into post-secondary programs (not UEP or Pre-Tech programs).
3. Students who were partially funded, attending during the previous year and who maintained a grade point average requirements toward program completion.

GROUP 2 - Categories to receive separate priority within the group:

1. Students in professional degree and pre-entry degree programs:
1.1 Students entering higher program levels (Masters, Ph.D.)
1.2 Combination degree programs, such as law, medicine.
2. Returning students, returning after a leave of absence for authorized medical or personal reasons, with grade point average requirements as the continuing students in Group 1.
2.1 Returning to the same program.
2.2 Returning to a different program.
3. UEP and/or Pre-Tech students.
3.1 Students with weak academic Grade 12.
3.2 Students with GED 12.
4. Mature students going directly into a post-secondary program.
4.1 Students without academic Grade 12, ABE 12 or GED 12.

GROUP 3 - These categories will be prioritized separately:

1. Students re-admitted after first notice of “Required to Discontinue” (RTD).
2. Students changing programs without completing any program because of poor grades and/or poor attendance.
3. Returning students in programs for several years without qualifying for a certificate.

7.0 Limits of Support:

7.1 Support will be provided for five levels of post-secondary education and limits are placed on the duration of support according to the level of the program:

Level 1 Technical Institute/Community College programs - 1 to 2 academic years, depending on institution’s normal program duration.
Level II University Undergraduate program - academic years based on the university’s normal program duration.
Level III University Professional (Pre-entry) Studies - duration of support will be in
Accordance with university’s normal program duration (law, medicine).
Level IV Post Graduate programs - duration of support will be in accordance with university’s normal program duration; however, reasonable limits may be established on the basis of individual need.
Level SC Duration of courses must be more than two months in length, but not longer than 8 months to full completion.

7.2 Mistawasis Post-Secondary Education Office will count student months which have been previously utilized regardless of whether a student received funding from another agency and who administered the PSSSP program:

Level 1: 10 months minimum to 24 months maximum for diploma
Level 2: 32 months minimum to 40 months maximum for degree
Level 3: 48 months maximum for Professional degree
Level 4: 18 months maximum for Masters/24 months for Ph. D. degree
Level SC: 2 months minimum to 8 months maximum

7.3 Students who transfer programs more than once at any level of funding will require approval of Post-Secondary portfolio councilor prior to approval of continued funding. Student is allowed only one opportunity to transfer from Level II to Level I prior to/or non-completion of Level II program.

7.4 Where students change programs within one of the funding levels, the academic time used for each program within this level will be counted for support purposes.

7.5 Students who have attended and completed a Level I program with support from Mistawasis Post-Secondary Program, are ineligible for further support at that level.

7.6 Students who have completed a Level II program with support from this program are ineligible for Level I program support.

7.7 Students who are eligible for financial assistance during Intersession/Summer Session must:

Provide a letter of support from department head indicating that classes are required toward student’s program completion by June, August or December of the current year and
Proof that student had successfully completed the previous academic year and
Provide letter of confirmation from the department head of program verifying that classes are offered only during Intersession/Summer Session and are required for program completion.

7.8 Level III funding arrangements will be dealt with on an individual basis and arrangements depend upon the program and institution, however maximum cumulative total years of support outlined in Section 7.2 will apply.

7.9 Funding will be considered when a student drops out of a Level II program and goes to Level I program. If student returns to Level II program before completing the Level I program, funding months at Level I will be counted toward their Level II eligibility for support purposes. Students who decide to choose this alternative will not be eligible for any further sponsorship at Level I or additional sponsorship (refer to Sec. 7.2) beyond their original number of eligible student months at Level II.

7.10 Students who become eligible for support and who have previously completed a portion of their program studies without support from Mistawasis Post-Secondary Education Office will receive support for the balance of their program of studies in accordance with Sec. 7.2.

8.0 Types of Support and Allowances:

8.1 Tuition and Book Support:
Tuition and book support will be provided to eligible students enrolled in any of the five levels. Any period during which a student received no allowance, but only support for tuition and books will not be counted as student months in terms of eligibility for full sponsorship (living allowance).
Student may be provided up to $500.00 per semester, $1000.00 per academic year for full time enrollment.
Includes student’s fees for registration, tuition, tutorials, initial professional certification and final examination fees.
Students enrolled in private institutions will be eligible for tuition support at the actual tuition rate where no comparable program is available at a public institution in Canada. The onus for showing no comparable program is available will be on the student.
When program is available in the province, and student chooses to attend out-of-province, Mistawasis Post-Secondary Education Office will pay tuition at the rate charged by local post-secondary institute offering the same program.

8.2 Part-Time Support:
When a student in attendance at a post-secondary institute and enrolled in less than 12 credit hours per semester will be considered for only tuition and book allowance.
Applicants who are employed and want to take a course that will enhance their chances of advancement in their current line of employment will be eligible for tuition and book costs.

8.3 Living Allowance:
Living allowance structure and entitlement (Appendix “A”).
To qualify for living allowance, each applicant must be selected and approved by a selection committee in accordance to Section 4.2a and c, and Section 6.
For continuation of living allowance, student must at all times be registered in 24 credit hours for the academic year or 12 credit hours per semester with no withdrawals or dropping classes.
Students will continue receiving full living allowances for the duration of their practicum, if it is part of their program structure for completion. Letter confirming practicum placement as a mandatory part of the program is required as soon as possible.

8.4 Relocation Support:
Upon program completion, students may be eligible for a $500.00 relocation allowance to move from their existing community to a location where employment has been obtained. The student must provide acceptable proof that confirms employment position. This allowance is a one time only basis.

8.5 Transportation:
Sponsored students who elect to commute daily from the reserve, rather than relocate, will not be eligible for extra funding to help cover the costs of daily transportation.

8.6 Assistance for Special Needs:
Special transportation costs will be paid by Mistawasis Post-Secondary Education Office for sponsored students with disabilities.
Students whose dependent has a disability will be provided extra assistance. Each request will be dealt with on an individual basis.

8.7 Tutorial Costs:
Where tutoring costs are not part of the student’s tuition costs, these students can access 20 hours per semester for tutoring services and Mistawasis Post-Secondary Education Office will pay the cost at $200.00 maximum. If extra tutoring is require above and over the limited amount, student needs to contact the Post-Secondary Coordinator.

8.8 Contingency/Emergency Assistance:
Student funded through other agencies and whose funding is being detained, may be eligible for financial assistance. Each request will be dealt with individually and the amount of assistance granted will vary in each case. This is a one time only basis.

8.9 Emergency Travel Assistance:
Student will be provided emergency travel assistance when
death occurs in the immediate family (refer to Sec. 3.12, 3.13)
member of immediate family is in an extremely critical condition in hospital.

Each request will be dealt with individually and the amount of assistance granted will vary in each case.

8.10 Graduation Incentive:
Letter of confirmation is required from the institute that indicates the month of program completion and convocation. The graduation incentive rates are as follows:
a) Certificate program (8 months or over) $250.00
b) Diploma program (2 years or less) $250.00
c) Degree program (3 year) $750.00
(4 year) $1000.00
These are non-cumulative and are one-time incentives upon confirmed completion.

8.11 Payment of other fees:
Payment of fees are limited to one payment a year per member - application/admission fees, and transcript fees (secondary or post-secondary).

9.0 Obligations of Administering Organization:

9.1 The Post-Secondary Coordinator will provide PSSSP policy seminar for first year students at the nearest convenient center during the month of September.

9.2 The Mistawasis Post-Secondary Program will not accept responsibility for any financial commitments made by a student prior to applying for and receiving final approval of sponsorship from Mistawasis. Also, it will not accept responsibility for students who refuse to access available professional services that will benefit the student.

9.3 The Post-Secondary Coordinator will meet with students’ instructors/academic counselors during the months of October and February, to determine which students will need to access extra services that will enhance student’s level of success. The instructor/academic counselor recommendations will be made known to the student by letter. Follow-up of recommendations will take place November and March. If student wishes not to comply with the recommendations set forth, funding will be suspended until other terms are worked out that are acceptable to all parties concerned.

9.4 All students approved for sponsorship will be mailed information packages and confirmations of sponsorship will be faxed to the institutions.

10.0 Accountability of Administration:

10.1 Every effort will be made by Mistawasis Post-Secondary Education Office to recover over payments of students who misuse funding by not fulfilling all the terms of the Mistawasis PSSSP policy.

10.2 Every effort will be made by Mistawasis Post-Secondary Education Office to recover overpayments of students who misuse funding by misrepresenting their marital, dependent and/or program status on application.

10.3 When misuse of post-secondary funds is discovered, Mistawasis Post-Secondary Education Office will adopt the following procedures:
write a letter to the student stating the findings,
if student is, in fact, found to be a misuser of funding, Mistawasis Post-Secondary Education Office will suspend student from further funding or take other corrective action,
arrangements satisfactory to Mistawasis Post-Secondary Education Office for repayment by the student will be a condition of any future funding.

11.0 Obligations of Funded Students:

11.1 It is understood that the PSSSP is intended to provide support for students with serious intent to succeed in post-secondary education. Expectations are that the students will attend classes regularly, submit assignments as required by programs and generally apply themselves to their studies. Students are expected to conduct themselves accordingly.

11.2 A student contract, referred to as Appendix “B”, must be signed and returned along with your official class registration form to the Post-Secondary Education Office by August 24 for the September cheque to be issued.

11.3 Students will be dealt with fairly and equitably under the PSSSP and are expected to diligently apply themselves to their duties as students.

11.4 Students receiving living allowance are required to submit the following to Mistawasis Post-Secondary Education Office:
- course outline for each class with instructor’s name and phone number before September 15 and January 15,
- academic profile (classes taken/classes needed for program completion) before September 15 and second profile to follow before January 15,
- signed declarations before the 15th of each month of funding, beginning September through to April.

11.5 In the event that the student, through his/her wrongdoing, indicates a lack of success in his/her studies, and if this opinion is borne out through the institute being attended, it may be necessary to reallocate financial resources by terminating the student’s support and applying the funds in areas showing greater promise of success.

11.6 The Mistawasis Post-Secondary Coordinator is authorized by this policy to discontinue provision of monthly living allowance to any student who is identified as not meeting:
- the minimum achievement requirements,
- dates specified for submission of required documents (refer to Sec. 11.2, 11.4),
- students who fail to provide transcripts prior to following dates:

January 25 - Fall semester transcripts
May 25 - Winter semester transcripts
September 25 - Spring/Summer semester transcripts

11.7 It is the responsibility of the student to notify the Mistawasis Post-Secondary Education office in writing of any changes in address, phone, marital status, dependents, income, etc., as soon as possible.

11.8 Students will be responsible for paying all program costs from which they withdraw for reasons other than certified medical reasons. Reasons for medical withdrawal must be acceptable to the institution the student attends. In the event of unauthorized withdrawals, funding will be discontinued (refer to Sec. 8.3b, c), payments must be recovered before additional funding will be considered.

11.9 Incomplete classes: if student fails to receive credits for class(es) during a semester, tuition costs will be recovered from student’s allowance (refer to Sec. 12.8).

12.0 Accountability of Funded Students:

12.1 Every effort will be made by the Mistawasis Post-Secondary Program to recover living allowances provided to students who misuse funding by not fulfilling the terms of the student contract or who misrepresent their marital status, dependents, academic or program status on application. In all levels of sponsorship, recovery of expenditures will be limited to living allowances provided to the student during the semester the drop out occurred, with no reduction in the number of months for which the student was eligible for.

12.2 When misuse of funding is discovered, the Mistawasis Post-Secondary Program will adopt the following procedures:
write a letter to the student stating the findings;
if student is, in fact, found to be misusing funding, Mistawasis Post-Secondary Program will suspend that student from further funding or take other corrective action.

12.3 Mistawasis Post-Secondary Program will arrange to recover expenditures made on behalf of a student who drops out of a fully funded program, without a valid, substantiated reason. Recovery will be made through deductions from eligible living allowance as a condition of future funding, should the student again apply for sponsorship.
After having dropped out of a fully funded program, any applicant who does not agree to a recovery plan will automatically disqualify him/herself from full sponsorship.
A student who drops out of a fully funded program will be required to wait for a minimum of four years before being considered for full sponsorship (living allowance). Such a student will not qualify for partial sponsorship (tuition & books) for at least four years following the semester during which the dropout occurred.

12.4 Students who initially provide proof of registration in 12 credits in order to qualify for living allowance and who subsequently drop one or more classes without a valid reason substantiated by the institute will be responsible to cover the costs of these classes. Furthermore, as a result of dropping classes, the student no longer meets the criteria requirements to qualify for a living allowance for the balance of that semester or academic year.

12.5 Students who are required to discontinue are not eligible for full sponsorship for a period of four years from the time the “RTD” status was applied. This also applies to “Withdrawls” because of low academics (marks) or lack of attendance, as determined by the Institute or by the Mistawasis First Nation. Where a student has been “RTD” a second time, that student will be eligible only for tuition and books for the remainder of their academic pursuits.

12.6 Students will be allowed only one valid and substantiated medical deferral, which will not require Councilors with Post-Secondary Portfolio’s approval for continued sponsorship. More than one request for complete medical deferral of all examinations for that semester will be subject to approval for continued sponsorship. Students must provide Mistawasis Post-Secondary Education Office with all appropriate documentations with their request for a medical deferral.

12.7 In the case of clear abuse, students will be disqualified from full sponsorship (living allowances) for a period of four years, after which time recovery of living allowances will be a condition of continued sponsorship. If student refuses recovery of living allowances, he/she will be eligible for only tuition and book costs.

12.8 Living allowance repayment schedule:
$500.00 or less, payment recovered by end of current semester;
$500.00 - $1500.00, payment recovered by end of current fiscal year,
over $1500.00, sponsorship discontinued. Student has the right to appeal (refer to Sec. 15).

12.9 Student will be responsible for the cost of exam rewrites.

14.0 Grievance Procedures:

If a student has a grievance regarding post-secondary education matters other than funding, the grievance procedure shall be as follows:

14.1 The student submits by registered mail a written report and supporting documentation to the Councilor(s) with Post-Secondary portfolio. The student will send copies to any other party they feel requires knowledge of the situation, expecting appropriate confidentiality will be maintained.

14.2 If the grievance is not resolved to the satisfaction of the student, the Councilors must submit the grievance to the forum of the Mistawasis Chief and Council. A decision, which is final for all parties, will be made at their next scheduled meeting.

15.0 Funding Appeals Process:

15.1 Every student has a right to appeal Mistawasis First Nation #103 Chief and Council’s decision regarding funding, however when a student application has been refused because available funds are fully committed, this appeal process will not be considered.

15.2 If misuse of funding (i.e., unreported Canada Student Loan, Provincial/Band Social Assistance, EI benefits, spouse’s employment income) is suspected, the student will be notified in writing. If there is no response, funding will be terminated immediately.

15.3 Pre-Appeal process:
Firstly, the student must discuss the matter of concern first with the Post-Secondary Coordinator;
Secondly, if issue is still unresolved, a meeting with the student, Post-Secondary Coordinator, and portfolio councilor must take place immediately or shortly thereafter;
Lastly, if issue is still unresolved, student can proceed with an appeal.

15.4 Procedures of the appeal process:
Notice requesting an appeal must be provided via registered mail to the portfolio councilor, in writing, within 10 days of receipt of the decision that is being appealed.
An appeal board consisting of 4 persons selected at large by the Post-Secondary Education Office must convene to hear the appeal within 10 business days of receiving the request for the appeal.
Student has a right to attend hearing in person or by a designate, but no professional counsel will be permitted at appeal hearings.
All costs of the appeal shall be paid for by the student.

15.5 The Appeal Board’s decision is final and binding on all parties.

15.6 Procedures of this whole Section 15 must be strictly adhered to when appealing. Once the appeal process is initiated, the student cannot request a meeting with the Mistawasis First Nation #103 Chief and Council.

16.0 Student Registry:

16.1 The Mistawasis Post-Secondary Education Office will maintain documentation with respect to student identifying information, academic records and support provided. Disclosure of academic information requires the informed consent of the student. Personal information is subject to the terms of the privacy provisions.

16.2 The Mistawasis Post-Secondary Education Office will maintain a student registry for statistical submission purposes to INAC Education Branch in Ottawa as required by their Education Data and Information System.

17.0 Policy Review:

17.1 There shall be a policy review annually, if necessary.

17.2 Policy review procedures shall apply except that student representation at the annual review will be sought. Representative shall be chosen by fellow students.

Maximum Monthly Allowance

A. Single student $1200.00

B. Single/married student with:

- one dependent $1,210.00

- two dependents $1,260.00

- three dependents $1,310.00

- additional dependents $75.00 each

APPENDIX “B”
STUDENT CONTRACT

I understand the following conditions apply to my sponsorship by the Mistawasis First Nation #103 for post-secondary studies:

1. I will accept the responsibility to adhere to the institute regulations and meet all the standards required by the institute and program of studies for continuation in my course of studies.

2. I agree to attend all classes as scheduled by the institute and/or my program of study.

3. I agree to consult with my instructor(s) of my program if any problems arise academically.

4. I agree to consult with a counselor if any problems arise emotionally, physically, or financially. This means the on-campus counseling program that is available for students; a signed declaration is also required.

5. I agree to provide all required documentation to the Mistawasis Post-Secondary Education Office in accordance with the Mistawasis Post-Secondary Policy.

6. I agree to report in writing any changes to my student and/or program status promptly. I understand that it is a serious matter to provide false information and/or fail to report any changes in the information provided.

7. I authorize the Mistawasis Post-Secondary Coordinator to obtain information from persons, agencies or organizations to determine and/or verify my eligibility for benefits or services as outlined in the Mistawasis First Nation #103 Post-Secondary Policy.

8. I declare that all the information provided is true and complete and I make this solemn declaration believing it to be true and knowing that it is of the same force and effect as if made under oath.

9. I understand that I have a right to appeal any decision made with respect to my sponsorship in accordance with Mistawasis First Nation #103 Post-Secondary Policy.
